PetsDiag Lifeline Diag Sp. z o.o. Panel Regulations
A person who has performed at least one EHAA hair strand test at LifelineDiag Sp. z o.o. shall have the right to use the PetsDiag Panel via www.petsdiag.com, which provides functionalities defined herein.
The user shall be authorised to use the PetsDiag Panel aby a result of the test that the user receives after the test is performed by Lifeline Diag Sp. z o.o. Before or after the receipt of the result, the user shall create a PetsDiag Panel account at www.petsdiag.com.
In order to create a PetsDiag Panel account, the user shall provide a login in the form of their
e-mail address provided in the questionnaire included in the testing kit, and any selected password. The account activation link shall be sent to the e-mail address provided in the questionnaire referred to above.
In order to log into the PetsDiag Panel, the user must enter the login and the password.
When the user logs into the PetsDiag Panel, they can:
view their data entered in the system;
view the result of the EHAA test(s) performed;
order a written interpretation of the result;
discuss their result with a Lifeline Diag Sp. z o.o. consultant;
add results of other tests than EHAA.
receive personalised guidelines regarding dietary changes or supplementation, g. receive detailed dietary recommendations.
Recommendations provided by the consultant shall be of informational nature and the user shall decide whether to apply them or not. The recommendations are based solely on the information provided by the user, therefore it is likely that a Lifeline Diag Sp. z o.o. consultant may miss some necessary information, which might make their recommendations incomplete. Given the above, Lifeline Diag Sp. z o.o. shall not be liable for any consequences of the recommendations applied by the user.
Lifeline Diag Sp. z o.o. shall not be liable for operations performed at the PetsDiag Panel by third parties using the user’s login and password, which were improperly protected by the user. In such an event, you should immediately notify the incident to Lifeline Diag Sp. z o.o. by phone at: 533 356 340 or by e-mail at: email@example.com. Upon user identification, their access to the PetsDiag Panel shall be blocked.
Lifeline Diag Sp. z o.o. shall keep and disclose information provided by the user in a manner compliant with the law and information security policy of Lifeline Diag Sp. z o.o.
Any amendments to these Regulations shall enter into force on the date of their posting at www.petsdiag.com.
Secure login to the PetsDiag Panel of Lifeline Diag Sp. z o.o. shall be provided solely at www.petsdiag.com. Every registered user should make sure every time whether the website address provided in the browser window is correct. You should never attempt to log in using any links sent by e-mail or applications unauthorised by Lifeline Diag Sp. z o.o.
Lifeline Diag Sp. z o.o. shall not be liable for any IT tools enabling to connect to the PetsDiag Panel, which were not authorised by it. We discourage you from using such tools to connect as they may attempt to take over your user login or password and thus enable unauthorised persons to access your data.
